Your initial appointment will consist of a comprehensive dental and oral examination with consultation explaining your current dental status and as well as potential treatment options. A complete medical history, oral examination, condition diagnosis, and discussion of means of treatment will be accomplished in the first, or New Patient visit. A second appointment to provide treatment or to further discuss involved treatment options will be scheduled for another day.
Please assist us by providing the following information at the time of your consultation:
- A referral note and any x-rays, if available, from your referring dentist.
- A list of any medications you are currently or habitually taking.
- If you have medical or dental insurance, bring the necessary information. This will save time and allow us to help you process any claims.
Please alert the office if you have or have had a medical condition that may be of concern prior to any dental treatment (i.e., diabetes, high blood pressure, artificial heart valves, artifical joints, etc.), if you are consistently taking any medication (i.e., heart medications, blood pressure medications, aspirin, anticoagulant therapy, etc.), or if you are allergic to any drugs or medications.
If your previous dentist has recently taken x-rays, you may request that they forward them to our office. If there is not enough time prior to your appointment, please pick them up and bring them to our office for your consultation. Often copied or electronically transferred images are non-diagnostic and additional x-rays are necessary to allow a complete and accurate examination. If additional films are necessary, they can be taken at our office for you
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
